AUDIT SHEET Permit Use Form THESE ARE RESTRICTED USE DOCUMENTS Use of any of these audit forms requires a permit. I am requesting a permit to use the documents checked on the order sheet that follows. I will pay a onetime permit fee for the use of each audit form ordered. This onetime fee allows me to download and print permitted items for my own use, accessing each document a total of 3 times over a period of 3 days. I agree not to transfer the use of any audit form or permit to any person or corporation or offer it for sale. I guarantee the fee payment for use of the forms. I agree to pay a $20.00 fee plus any incurred financial institution fees if the payment is not deemed fully negotiable and honored by any financial institution. I agree that I will not stop payment or allow any merchant chargebacks on payment for the permit(s). I agree to defend this agreement. The permit is issued on a one time basis to each permit holder. I understand that the fee payment and charges are not refundable. The aspects of access and possession prohibit “refunds” for the permits and audit forms. Warning: No part of these publications may be reproduced or transmitted in any form or by any means, electronic or mechanical, including photocopy, recording, or any information storage and retrieval system without the written permission of the authors. Unauthorized possession and/or use of this use permit and material may be deemed a violation of patent and/or copyright laws of the United States and punishable. ‘Fair use’ publication is noticed and claimed by the author. The audit form method provides a bench mark or framework from which to examine dysfunctional practices and patterns occurring at various levels of participation in community affairs. Users of the audit forms must realize that making determinations, disclosing and publishing information they discover, write or attach to the audit form may set off reasonable responses, hostile attitudes and actions and/or insane delusions. Reactions may be in the form of aggressive behaviors, litigation, public perception management campaigns, or mere discounting the user’s value or purpose. The permit holder bears separate and ultimate responsibility for using the audit forms under any circumstance. Do not “weaponize” the audit forms against mere personal, social and political differences. Use the audit forms as a prudent and reasonable person would do under the same circumstances. This means protect the documents, their use, from misuse, such as deceitful acts, deceptions, disinformation, misinformation, etc., by persons not permitted to use the audit forms. There are no implied or expressed warranties on the use of these products except as provided for in writing on the face thereof.
